The purpose of these modules is to:
- provide an introduction to trends in youth psychiatric hospitalization
- describe the process for developing and the structure of the BIRCh Hospital to School Transition Protocol
- review steps for implementing the Hospital to School Transition Protocol
- utilize four case studies to highlight implementation of this protocol in practice.
Implementation of the hospital to school transition process is best supported by ongoing coaching and technical assistance, including learning from other schools and districts who have engaged in this work.

Complete the Registration Form
We will monitor your completion of the modules using our learning management system and contact you to follow up to complete the posttest questions after you have viewed the entirety of the module series. Once you have viewed the modules and completed the post-test questions with 80% accuracy or higher, we will provide a certificate of completion you can put towards your PDPs.

This four-page document is to be filled out upon student transition from hospitalization. Stakeholders needed to collaboratively complete this document include the school-based multidisciplinary team, hospital staff, community-based behavioral health providers, student, and family/guardians.
